What incident reporting software should do
Incident reporting software lets staff report what happened, routes the report to the right people, supports an investigation proportionate to the harm, turns findings into actions, and shows whether those actions worked. A system that only stores the report has done the smallest part of the job.
Where record-only systems fall short
- Reporting is slow. Long forms put front-line staff off, so events go unreported.
- The story is split. The incident is in one place, the risk in another, the actions in a spreadsheet and the policy in a folder.
- Actions drift. Nobody is chased, and closing an action just means someone ticked a box.
- Learning is lost. The next investigation does not benefit from what was found last time.
- Trends arrive late. Reports are built by hand, so patterns show up after the damage is done.
The five stages
- Report. A quick, clear way to say what happened, ideally in the reporter's own words.
- Triage. Decide how serious it is, who must be told, and what response is proportionate.
- Investigate. Find the root cause and contributing factors, using the timeline and the evidence.
- Act. Assign actions with owners and deadlines, and chase them.
- Prove. Check that the actions worked, record the learning, and watch for the problem coming back.
Questions to ask a supplier
- How long does it take a nurse or care worker to report an incident?
- Can an incident link to the risk, the control, the policy and the actions it relates to?
- Who verifies that an action worked, and is that recorded?
- What does the AI do, what does it show, and who decides?
- Is there a tamper-evident history of every change?
- What does it not do? For example, does it file with regulators for you?
How CGRC AI approaches it
Staff describe what happened in plain words and the AI drafts the form for them to check. For health and social care, the record carries harm levels, duty of candour, safeguarding, notification flags and the other governance questions regulators commonly ask. During investigation the AI reads your own policies and earlier similar closed cases and shows which it used. Actions are chased, can be verified by someone other than the owner, and stages can have target times with escalation. CGRC AI records and tracks, and submissions to national systems are still made through the national routes.

Common questions
What is incident reporting software?
Incident reporting software is a system that lets staff report incidents and near misses, then manages the investigation, the actions and the learning that follow.
Why not use a spreadsheet?
A spreadsheet cannot chase actions, control who sees what, keep a tamper-evident history or link an incident to the risks, controls and policies it relates to.
Does incident reporting software file reports with regulators?
Some can, but many only record. CGRC AI records and tracks reporting decisions and does not file reports with regulators for you.
