The idea
One complete governance system
Not a handful of separate tools that never quite talk to each other. CGRC AI covers every part of governance in one connected system, from information governance and data privacy to clinical governance, safety, people, funding and the board.
Governance
The rules, evidence and oversight that a board or a leadership team relies on.
Risk and safety
Understanding what could go wrong, and learning properly from what did.
Compliance and people
The duties you owe to the people you serve, your staff and the law.
Business
The agreements, funding, continuity plans and commitments that keep the organisation running.
Reports and dashboards look across all of it, so leaders see the whole picture and not one corner of it.
The problem
Most systems record. Few help.
Incidents sit in one place, risks in another, actions in a spreadsheet and policies in a folder. Someone has to chase every thread, and the lessons are easy to lose. CGRC AI keeps it all connected, so the next person to open a record can see what happened before, what was done, and whether it worked.
- 01
See problems sooner
Patterns across your own incidents, risks and audits show up early, so you can act before something happens again.
- 02
Make fixes stick
Every action has an owner and a date. It is only finished when someone else has checked that it worked.
- 03
Be ready when asked
Evidence is kept as you go, so a board, a regulator or a funder gets a clear answer without a scramble.
How it works
AI at every step
The AI is part of the everyday work, not a separate tool. It helps at each stage, and it always shows what it used.
Report
Describe what happened in your own words. The AI drafts the form and you check it.
Investigate
The AI reads the record, your own policies and your earlier similar cases, and tells you which ones it used.
Act
Assign actions, set deadlines, and be told when something is overdue or stuck.
Prove
Show what changed and that it lasted, with a full record of who did what and when.
The AI drafts and suggests. People always decide.
Who it is for
Set up for the questions you are asked
Health and social care
Hospitals, clinics and care providers. Incident, safeguarding and complaint handling shaped around how clinical teams really work, with the questions regulators commonly ask built in. CGRC AI records and tracks, and national submissions are still made through the national routes.
Charities and community organisations
Trustees and managers who need to show they are in control. Safeguarding, serious incident decisions, conflicts of interest and board records in one place. A decision not to report is recorded as carefully as a decision to report.
Life sciences and research
Sponsors and sites running clinical research. Oversight of trials, protocol deviations and monitoring visits, with approvals that need the signer to prove who they are at the moment they sign. For governance and oversight, not for holding trial participant data.
Who is behind it
Built by people who do this work
CGRC AI comes from Cognito Consultants, a governance consultancy. Our team has spent years working in healthcare governance, from the front line to senior leadership, using and configuring systems like this one for hospitals. We know where the old systems frustrate people, and we built this to fix that.
Honest about where we are
- CGRC AI is at pilot stage, and we are working with a small number of organisations.
- It records and tracks. It does not file reports with regulators for you.
- The AI is built on your own records. It is not trained on one customer's data and shown to another.
- We hold no formal certifications yet, and we will say so on this page when that changes.

We are looking for a small number of organisations to try CGRC AI properly and help shape it. You get hands-on support from the people who built it, and we get honest feedback.
Or use the contact form, or write to info@cgrcai.com, and we will arrange a conversation.
